The Licensing Inspector conducted an investigation in response to a self report incident of supervision on July 18,2019. Interviews were conducted with staff and the classroom was observed. The information gathered during this investigation supports the allegations; violations were discussed with the center director. Standard #: 22VAC40-185-340-F Description: Based on staff interview, children under 10 years of age were not within sight and sound supervision of staff. Evidence: On Thursday, July 18, 2019 staff #1 observed 7 children, ages 6-12 years old alone in the classroom. Staff #2 stated to staff #3 that she was taking a few children to put on their sunscreen. She took the children out the classroom door to the front of the building. Staff #2 said she was gone about 5 minutes. Staff #3 said to staff #2 that he was taking a few children outside to apply sunscreen. he took the children out to the playground. Staff #3 said he was gone about 5 minutes. Both staff #2 and #3 went out separate doors, leaving children alone in the classroom. Staff #1 walked into the school age classroom around 10:25 am, she found 7 children, ages 6-12 years old inside the classroom. The children were playing a game on the carpet, playing in home living and one was sitting on the couch reading. Plan of Correction: We spoke to the teachers about proper communication. We will ensure that one staff always remains inside while staff are outside applying sunscreen. proper ratios will be maintained at all times.
