Comments:
An unannounced renewal inspection was conducted with the director beginning at 9:25 AM and ending at 10:40 AM. There were fourteen children in the care of three staff during the inspection. The children's ages ranged between three and five years old. The children were observed playing with blocks, cars, and dolls and coloring. Two children were observed eating morning snack which consisted of dry cereal and milk. Seven children's records were reviewed and in compliance. Five staff records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and weather-band radio were reviewed and in compliance. The emergency drill log was reviewed and in compliance. The most recent evacuation drill was documented as 7/12/19. The most recent shelter-in-place drill was documented as 5/10/2019. The most recent health inspection was dated 4/13/18. Description: Based on review of documentation, the center did not have annual approval from the health department as required. Evidence: The most recent health inspection was dated April 13, 2018.

Plan of Correction: The director stated that she has contacted the health department in order to have the inspection scheduled. The health department stated that a supervisor was made aware of the issue and will be contacting them back. The director stated that she will continue to follow up with the health department until the inspection is completed.
