Comments:
An unannounced monitoring inspection was conducted today from 1:00pm-2:30pm. There were 33 children directly supervised by 5 staff. The physical plant, 4 staff records, 5 children?s records,evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in coloring, watching a movie and sleeping. Teachers were observed playing quietly with awake infants while others napped. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-450-A
Description: Based on observation, cots used by children did not have linens consisting of both a bottom and top cover. Evidence: Seven children were observed sleeping on cots without a bottom cover.

Plan of Correction: I will ask the parents to bring in the bottom sheets.

Standard #: 22VAC40-185-570-E
Description: Based on observation, prepared bottles for infants were not labeled with the date. Evidence: A total of 6 bottles were observed without a label indicating the date.

Plan of Correction: We will retrain staff on requirements for labeling.

Standard #: 63.2(17)-1720.1-B-3
Description: Based upon documentation reviewed, an out of state child abuse and neglect result was not obtained. Evidence: One of the four staff records reviewed states that Staff 4 has lived out of the state of Virginia in the preceding 5 years, and no out of state child abuse and neglect results were obtained (Staff (4); DOH 06/2018).

Plan of Correction: We will request the central registry check.
