Technical Assistance:
**Discussed labeling of breakfast foods brought from home. **Discussed medication form that had dosage amounts that may be confusing to read (one measurement in weight, one in volume)

Comments:
An unannounced monitoring inspection was conducted today from 8:10am-10:30am. At the beginning of the inspection there were 54 children directly supervised by 7 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and being dropped off by parents. one staff member was observed comforting a child who was crying at drop off. Hand-washing procedures were also observed. There was a variety of age appropriate materials for the children and children's artwork was observed to be prominently displayed in the classrooms. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-280-B
Description: Based on observation, not all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: Four vials of essential oil labeled with the warning "Keep out of reach of children" were observed in an unlocked drawer in the Monkey's classroom.

Plan of Correction: We will lock these items up.

Standard #: 22VAC40-185-510-J
Description: Based on observation, medication was not stored in a locked place using a safe locking method that prevents access by children. Evidence: Medication was observed stored in a cabinet without the lock engaged. The current lock on the door is not designed to prevent access by children and when engaged would not meet the definition of a 'locking method that prevents access by children'.

Plan of Correction: The current lock was engaged during the inspection, and a new more secure lock will be added to the cabinet door.

Standard #: 22VAC40-185-510-N
Description: Based on a review of medication, two of five medications were present at the center for a period of more than 14 days past the expiration of the authorization. Evidence: 1. Child 1's medication was present at the center on 7/16/2019, the authorization expired 6/2019. 2. Child 2's medication was present at the center on 7/16/2019, the authorization expired 6/20/2019.

Plan of Correction: 1. Child 1's parent was notified by phone call and the medication will be sent home. 2. Child 2's parent was telephoned during the inspection and a new authorization form is being sent home with the parent's today.
