Comments:
An unannounced monitoring inspection was conducted from 12:30 - 3:00 pm with the center director. There were 40 children, ranging from four months to five years of age, present with seven staff supervising. Children were observed napping, waking, and eating afternoon snack. Snack served today was a cheese stick with crackers and water. Five children's records were reviewed and in compliance. Four staff records were reviewed. One medication/authorization form was reviewed and in compliance. The last fire drill was conducted: 6/10/19 and the last shelter-in-place drill was conducted: 5/16/19. Description: Based on a review of four staff files, and an interview with the director, it was determined that repeat tuberculosis (TB) test/screenings were not completed every two years. Evidence: The TB test for staff #2 is dated: 2/23/17.

Description: Based on observation, not all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: There was a bottle of bleach water solution left on a shelf above the changing table in the two's classroom. There was a small bottle of Dawn dishwashing detergent by the handwashing sink in the infant/toddler classroom. In the 4's classroom, there was an unlocked cabinet with shaving cream that stated, "keep out of reach of children." Pool chemicals were in an unlocked cabinet by the pool; no children were using the pool during today's observation.

Description: Based on observation, and interviews with staff and the director, not all infants were laid on their back to sleep. Evidence: a four month old infant was observed in the crib sleeping on her side.

Description: Based on observation, and interviews with staff, it was determined that rest cots used by children did not all have linens consisting of a top cover and a bottom cover. Evidence: in the 3's/4's class, approximately 8 children did not have a bottom cover and one child didn't have a top or bottom cover; in the 2's class, one child did not have a bottom cover.

Description: Based on observation, and an interview with the director, the center did not have written safety rules for swimming posted by the pool. Evidence: the list of pool rules is not posted by the pool.

Description: Based on review of four staff records, and an interview with the director, it was determined that not all staff had a completed sworn statement or affirmation before employment. Evidence: Staff #1 (date of hire: 2/6/2019) and Staff # 3 (date of hire: 6/19/19)did not have documentation of a sworn statement in their file.
