Comments:
An unannounced monitoring inspection was conducted from 10:45 am - 2:15 pm with the center director and staff. There were 73 children, ranging from three months to 11 years of age, present with 11 staff supervising. Children were observed listening to a story, participating in a marching band, playing outside on the playground and eating lunch. School age children were observed returning from a field trip to Waterworks. Lunch served today was fish sticks, peas, applesauce and milk. Seven children's records were reviewed and in compliance. Five staff records were reviewed. The emergency escape drill log was observed and in compliance. Three medications and authorization forms were reviewed and in compliance. Standard #: 22VAC40-185-160-C
Description: Based on a review of five staff files, and an interview with the assistant director, it was determined that repeat tuberculosis (TB) test/screenings were not completed every two years. Evidence: The TB test for staff #3 is dated: 7/8/2017.

Plan of Correction: Staff will be sent to get an updated TB test.

Standard #: 22VAC40-185-570-A
Description: Based on observation, not all children placed in high chair seats had safety straps securely fastened. Evidence: in the infant room there were four infants eating lunch in high chair seats, and none had the safety straps securely fastened.

Plan of Correction: Safety straps were fastened during the inspection.
