Comments:
An unannounced monitoring inspection was conducted between 11:00am -2:00pm. Upon arrival there were 80 children in care with 15 staff present. A sample of 2 staff and 8 children's records were reviewed, in addition to 2 medications and 1 over-the-counter skin product. Transportation was also reviewed with administration. School-age children were observed in the middle room doing activities with staff. Preschool children were observed during lunch and preparing for nap. Infants were observe sleeping in cribs, being held by staff and tummy time. A baby was observed eating lunch in a high chair.

Standard #: 22VAC40-185-60-A
Description: Based on a record review, the center did not ensure all required information was kept for each child enrolled. Evidence: Child #4 (start date 12/10/18) and #5 (start date12/20/19) did not have documentation of having the two required emergency contact information.

Plan of Correction: The following corrective action was provided by the licensee: Management has been retrained that ALL children have required emergency contacts noted during paperwork appointments. Children are not to start without this. DM will review for compliance quarterly.

Standard #: 22VAC40-185-260-A
Description: Based on a record review the center did not provide an annual fire inspection. Evidence: The center's last fire inspection was dated 04/10/18.

Plan of Correction: The following corrective action was provided by the licensee: Fire Marshall was contacted this same day. The director will calendar this 60 days in advance for next year to ensure compliance. DM will also calendar

Standard #: 22VAC40-185-290-2
Description: Based on observation, the center did not ensure cords were secured out of the reach of children. Evidence: In the school-age room and preschool room there were fans plugged into an electrical outlet. The cords posed a tripping hazard as they dangled from the fan that was sitting on a shelves.

Plan of Correction: The following corrective action was provided by the licensee: The cord s were secured the day of the visit. Management will ensure that all cords are secured daily, throughout the building.

Standard #: 22VAC40-185-440-I
Description: Based on observation the center did not ensure cribs were placed where objects outside the crib such as cords from blinds or curtains are not within reach of infants or toddlers. Evidence: A 13 month-old baby was observed standing in her crib. The crib was placed in the back corner of the room underneath the windows. The mini blinds were half up and the cord was dangling over the crib, in reach of the child.

Plan of Correction: The following corrective action was provided by the licensee: This child was moved to another crib, the day of the visit, and a child who cannot stand up was moved to that crib. Safety Captain will observe weekly for compliance as will DM during school visits.

Description: Based on observation the center did not ensure all required emergency supplies were included in the first aid/emergency kit. Evidence: The first aid/emergency kit that was on the van the center is currently using for field trips, did not have a working flashlight or batteries.

Plan of Correction: The following corrective action was provided by the licensee: The first aid kit was replenished the same day as the visit. Moving forward, the safety captain is to audit all first aid kits monthly for compliance
