Comments:
A complaint investigation was initiated on July 9, 2019 with another local agency and ended on July 11, 2019 regarding a two year old child with bruises on the leg. Interviews were conducted, and documentation and was reviewed. There was not a preponderance of evidence to support the allegation regarding child abuse and neglect. However, there are seven non-complaint related violations that are identified in the Violation Notice. Complaint related: No
Description: Based on record review, 1 child's record did not contain documentation of all required information. Evidence: Child #1's record did not contain documentation of an address for the second emergency contact person, and documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3. The most recent documented parental acknowledgement of up-to-date information was dated 1/20/2018.

Description: Based on a review of staff records, 1 of 4 staff members did not have the required staff records. Evidence: On 7/9/2019, the center did not have documentation of any of the required records for Staff B.

Description: Based on center observation, the center did not ensure that there is always an immediately available staff member, volunteer, or other employee with direct means for communication between the program leader and the staff member. Evidence: On 7/9/2019, from approximately 1:30pm-2:15pm, Staff A was in a classroom, counted in staff-to-children ratio, and there was no immediately available staff member with direct means of communication between the two of them.

Plan of Correction: We have re-worked the staff schedule so that there is always a manager available.
