Comments:
An unannounced monitoring inspection was conducted today from 10:20am-1:30pm. There were 40 children (infant-5yrs.) supervised by 10 direct care staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed having FitKids classes outside, blowing bubbles, and playing with Legos and Kelsey counters. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 of 4 children's records did not contain documentation of child updates and confirmation of up-to-date information in the child's record as required by 22 VAC 40-185-420 E 3. Evidence: Child #1's (start date: 9/28/2015) most recent documented parental confirmation of up-to-date information was dated 1/18/2018.

Plan of Correction: I will send mom an email and have her update today.

Standard #: 22VAC40-185-240-D-5
Description: Based on record review, the center did not ensure that there is always at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children. Evidence: There was no documentation of a staff member on duty with current daily health observation training.

Plan of Correction: A training is scheduled for August 13, 2019.

Standard #: 22VAC40-185-280-B
Description: Based on observation, 1 of 6 classrooms did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: The Two's classroom had 3 spray bottles containing cleaning chemical cleaning solutions were in the unlocked teacher closet.

Plan of Correction: Corrected during inspection.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, 1 of 3 staff records did not contain documentation of out-of-state central registry results. Evidence: Staff A's (date of hire: 4/29/2019) record did not contain documentation of completed central registry results for Delaware.

Plan of Correction: A follow-up was conducted today.
