Comments:
The licensing inspector conducted an unannounced monitoring inspection Friday, July 12, 2019 from 10:00am to 12:25pm. The children were observed playing with toys during center time, playing a card game of uno, and coloring pictures. Lunch consisted of meatballs, oranges, mixed vegetables, and milk. The lunch menu was posted. All classrooms and the playground were inspected. Seven children?s records and nine employee records were reviewed during this inspection. Medication is being administered and was reviewed. Last emergency drill: 06/13/2019 Shelter-in-place drills: 03/06/2019 Fire Inspection-06/25/2019 Health Inspection-06/25/2019 Total enrollment today: 67 Today, the following child to staff ratios were observed: Twos 1 ? 7:2 Twos 2-11:3 Older Two's-10:2 Preschool ?7:2 Pre-K ?8:1 Pre-K-14:3 School Age 1-10:2 School Age 2 ?off site on a field trip The violations from the previous inspections were checked for corrections. Description: Based on a review of children's records on 07/12/2019, the center did not ensure that each child shall have a physical examination before the child's attendance; or within one month after attendance. EVIDENCE: The record of Child #1, whose start date at the center was 08/27/2018, did not contain documentation of a physical examination.

Description: Based on a review of children's records on 07/12/2019 the center did not ensure that staff shall request at least annually parent confirmation that the required information in the child's record is up to date, such sharing of information shall be documented. EVIDENCE: 1. The most recent confirmation that the information is up to date in the record of Child #2 is dated 09/15/2017. 2. The most recent confirmation that the information is up to date in the record of Child #3 is dated 04/09/2018. 3. The most recent confirmation that the information is up to date in the record of Child #6 is dated 08/29/2016.

Description: Based on a review of medication on 07/12/2019, the center did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center. EVIDENCE: 1. The authorization for medication for Child #8 expired on 03/11/2019. 2. The authorizations for medications for Child #9 expired on 03/01/2019, and 05/22/2019. 3. The authorization for medication for Child #10 expired on 06/15/2019. 4. The authorization for medication for Child #11 expired on 05/04/2019.
