Comments:
The licensing inspector conducted an unannounced subsidy health and safety inspection inspection Friday, July 12, 2019 from 10:00am to 12:25pm to review supplemental health and safety requirements. The children were observed playing with toys during center time, playing a card game of uno, and coloring pictures. Lunch consisted of meatballs, oranges, mixed vegetables, and milk. The lunch menu was posted. All classrooms and the playground were inspected. Seven children?s records and nine employee records were reviewed during this inspection. Medication is being administered and was reviewed. Last emergency drill: 06/13/2019 Shelter-in-place drills: 03/06/2019 Fire Inspection-06/25/2019 Health Inspection-06/25/2019 Total enrollment today: 67 Today, the following child to staff ratios were observed: Twos 1 ? 7:2 Twos 2-11:3 Older Two's-10:2 Preschool ?7:2 Pre-K ?8:1 Pre-K-14:3 School Age 1-10:2 School Age 2 ?off site on a field trip A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety requirements were found. Please see the violation notice. Description: Based on a review of staff records on 07/12/2019, the center did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment. EVIDENCE: 1. The documentation of TB in the record of Staff #2, whose date of hire was 05/29/2019, is dated 01/14/2019. 2. The documentation of TB in the record of Staff #4, whose date of hire was 06/04/2019, is dated 09/22/2018. 3. The documentation of TB in the record of Staff #5, whose date of hire was 05/20/2019, is dated 09/07/2018. 4. The documentation of TB in the record of Staff #7, whose date of hire was 03/28/2019, is dated 04/17/2019.

Description: based on a review of staff records on 07/12/2019, the center did not ensure that all staff who work directly with children shall have within 90 days of the date of employment current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. The training shall include an in-person competency demonstration. EVIENCE: The CPR certification in the record of Staff #7, whose start date was 03/28/2019, did not have an in person competency demonstration. The certificate stated it was the online portion only.
