Comments:
An unannounced renewal study was conducted today with the Assistant Director, who was present with 39 children with four staff providing supervision. The three/four year old children were on a field trip to a local water park. The schoolagers were watching a movie and the younger preschoolers and infants were napping during the initial walkthrough. Six medications for five children were reviewed today as well as 6 staff and children's files. Eight injury/accident reports were reviewed as well. Health inspection: 10/30/18 Fire inspection: 1/17/19 Arrival time: 1 pm Departure time: 3: 30 pm

Violations:
Standard #: 22VAC40-185-510-N
Description: Based on an inspection of six medications, it was determined that a prescription medication for one child had expired and the authorization was not renewed or picked up by the parent within 14 days nor disposed of by the center. Evidence: A parent authorization was dated 6/14/19 for Ketotifen Fumarate solution for child A. The parent did not renew the authorization every 10 work days or picked up the medication within 14 days nor did the center disposed of by the medication.

Plan of Correction: We contact the parent and she will pick up the medication today.
