Technical Assistance:
Walls and doorways can obstruct sight and sound supervision and anything that delays at teachers access to a child in need can present a supervision issue. Consultation provided on documenting the time parents were notified of injuries to their child. s

Comments:
An unannounced renewal study was conducted with the director/owner. Thirty five children were present with five staff providing supervision. Most of the children were napping during the walkthrough. For lunch, the staff reported thats the children were provided black beans and rice, mixed vegetables, pineapple and milk. There were no medications on site. Six staff and eight children's files were reviewed today and determined to be in compliance. Fire inspection: 1/10/19 Health inspection: 5/28/19 Arrival time: 12:20 pm Departure time: 2: 35 pm

Violations:
Standard #: 22VAC40-185-80-A
Description: Based on observation and a review of the attendance log, it was determined that the center did not keep an accurate attendance of the children in attendance. Evidence: There were 8 schoolagers present however the attendance listed 11 children as being present.

Plan of Correction: We will retrain all staff on the need to make sure the attendance accurately reflects the children that are present at any given time.

Standard #: 22VAC40-185-340-F
Description: Based on observation, it was determined that children under 10 years of age are not always within actual sight and sound supervision of staff. Evidence: The Licensing Inspector observed one teacher supervising two classrooms at nap time by standing in the middle of the toddler and preschool classrooms. The teacher was observed going back and forth between the two classrooms, consequently leaving either the 3 toddlers or the 6 preschoolers out of sight and sound supervision during nap time.

Plan of Correction: Within 20 minutes a teacher returned from break and both classrooms were within sight and sound supervision of staff. We will either combine the two classrooms at nap time or we will make sure teacher are in both classrooms.
