Comments:
An unannounced focused monitoring inspection was completed on 07/22/2019 from 3:15 P.M.-5:30 P.M. The purpose of the inspection was for determining compliance to previous violations. There were 49 children in attendance with fifteen staff, and one administrator present. Three staff records, three children records, three medications and required documentation were reviewed, and observations in the classrooms and in the gym took place. The children were observed during snack time, handwashing, free choice activities, infant feeding, diapering, gross motor indoor play, behavioral guidance, and departure. Exceptions to compliance are noted on the violation notice. One repeat violations was sited.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on interview and review of medications, the center failed to ensure the care and protection of a four year old child.

Evidence:
1. During review of medication and required documentation Child 1M had authorizations for two emergency medications. One of the emergency medications was in the lock box in the office, the other emergency medication was housed in Classroom A. When Staff #3 was asked about the emergency medication she was unaware that there was a child that had an emergency medication in the classroom.
2. Staff #3 was alone with Child 1M from approximately 3:24 P.M.-4:30 P.M., and did not know where the emergency medication was located.
3. Staff verified that she did not know where the emergency medication was located.

Plan of Correction: Administration will meet will all staff including substitutes and make everyone aware of where all emergency medication is located.
