Comments:
A Subsidy Health and Safety inspection was conducted on 07/29/2019 from 9:30 A.M.-2:15 P.M. to review supplemental health and safety requirements. At the time of inspection 17 children were in the care of four staff and one administrator present. Five children records, six staff records, one over the counter medication and required documentation, six injury reports, emergency plan, first aid kits, emergency supplies, and posted required information were reviewed. Children were observed during free choice activities, large group literacy activity, art project, outside play, music and movement, storytime, handwashing, lunchtime, and naptime. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. The exceptions to compliance are noted on the violation notice. Description: Based on review of staff files, the center failed to obtain documentation of a negative tuberculosis screening prior to coming into contact with children. Evidence: 1. Staff #2's tuberculosis screening was dated 05/22/2019. Start date for Staff #2 was 05/06/2019. 2. Staff verified the date of the tuberculosis screening and the start date for Staff #2.

Plan of Correction: In the future administration will make sure that all staff have a tuberculosis screening or test before coming into contact with children.
