Technical Assistance:
Technical assistance was provided regarding spreading the mulch on the playground, there is enough mulch, just needs to be spread out under the fall zones.

Comments:
An unannounced monitoring inspection was conducted today from 9:00am-12:30pm. There were 113 children (infants-school age) supervised by 21 direct care staff. The physical plant, programming, 10 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, table games, water play and outside time. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 22VAC40-185-130-B
Description: Based on review of 10 children's records, 2 do not have updated documentation of additional immunizations every six months for children under the age of two. Evidence: Children #3 & 5 are under the age of two and do not have documentation on updated immunizations.

Plan of Correction: I will contact the parents and get the updated immunizations and review the records to ensure compliance.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center are not being maintained in a safe, operable condition. Evidence: The fence along the back area of the playground is broken, leaving nails protruding that could entangle clothing or snag skin.

Plan of Correction: I will contact maintance and have them repair it.

Standard #: 22VAC40-185-290-3
Description: Based on observation, not all electrical outlets have a protective cover. Evidence: An outlet in the Bears room was uncovered and accessible to children.

Plan of Correction: Corrected during the inspection, and we will make sure the staff does a morning review of the outlets.

Standard #: 22VAC40-185-420-E-3
Description: Based on review of 10 children's records, 5 records do not have documentation of annual update of emergency contacts. Evidence: Children #1, 2, 4, 6 & 7 do not have documentation of an annual review of emergency contacts.

Plan of Correction: I will contact the parents to get the updates to their emergency contacts and review all files to ensure compliance. We do our annual reviews in September.

Standard #: 22VAC40-185-570-E
Description: Based on observation, not all bottles are dated and labeled with the child's name and current date. Evidence: A bottle in the infant 2 classroom refrigerator was labeled with the child's name, but not the current date.

Plan of Correction: Corrected during the inspection, and staff will remind parents to name and date the bottles every morning.
