Comments:
An unannounced monitoring inspection was conducted today from 12:15 pm until 3:00pm. There were 30 children present with 8 staff member's at arrival. The physical plant, children's records, staff records, evacuation plans, emergency response plans, menu's, medications and emergency supplies were inspected. The children were observed, interacting with staff, coloring, completing lunch, napping, toileting and hand washing. Areas of non compliance are cited on the Violation Notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of 5 children's records, 4 of 5 records reviewed were incomplete. Evidence: Child #1 and #5's files are missing the addresses of both emergency contacts. Child #1, #2 and #5's file are missing documentation of the names of persons authorized to pick up the child. Child #2 and #3's files have health forms missing the parent signature. Child #5's file is missing documentation of place of employment for both parents.

Plan of Correction: I will get information from parents and put in each child's file

Standard #: 22VAC40-185-270-A
Description: Based on observation, there are areas of the center which are not maintained in a safe condition. Evidence: 1. On the fence close to the back door, there is a slat that is broken and it has nails sticking out which could entangle clothing or snag skin, 2. The gate near the swings on the playground has a broken slat with protruding nails that could entangle clothing or snag skin. 3. Areas of the fence toward the back of the playground and separating the center from adjacent property has broken slats that could endanger students.

Plan of Correction: This will be taken care of this weekend.

Standard #: 22VAC40-185-510-J
Description: Based on observation, the center failed to ensure that medication will be kept in a locked place using a safe locking method that prevents access by children. Evidence: Medication was in cabinet in a locking box, but the medication box was not locked.

Plan of Correction: I will keep medications locked up.

Standard #: 22VAC40-185-550-D
Description: Based on review of evacuation drill logs, the center failed to ensure that a monthly practice evacuation drill will be conducted. Evidence: The center did not provide documentation of a monthly practice evacuation drill for June 2019.

Plan of Correction: I will do drills monthly.
