Comments:
An unannounced monitoring inspection was conducted on July 24, 2019 from approximately 9:20 am to 1:00 pm. Five staff were present with twenty-six children. Five children and six staff records were reviewed. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. Description: Based on review, facility did not ensure 1 of 5 children's records has a physical examination within a month of beginning attendance. Evidence: The record for child #5 (start date of attendance February 19, 2019) did not have a physical. Owner confirmed missing information.

Description: Based on review, facility did not ensure 2 of 6 staff have a negative TB test or screening within 21 days of beginning employment. Description: Based on review, facility did not ensure 6 of 6 staff records have documentation of required information. Evidence: The following was found during review of staff records: 1. The record for staff #1 (start date of employment January 21, 2019) did not have two reference checks and emergency contact information. 2. The record for staff #2 (reported start date of employment January 21, 2019) did not have start date of employment, two reference checks, emergency contact information and orientation training. 3. The record for staff #3 (reported start date of employment June 17, 2019) did not have start date of employment and emergency contact information. 4. The record for staff #4 (start date of employment June 17, 2019) and staff #6 (start date of employment March 26, 2019) did not have emergency contact information. 5. The record for staff #5 (start date of employment March 6, 2019) did not have emergency contact information and orientation training. 6. Evidence: There was not a posted daily schedule for any of the classrooms. Owner stated the daily schedule had been posted on the front parent board but it had been taken down.

Description: Based on observation and interview, facility did not ensure there is a provision made for an individual place for each child's belongings. Evidence: The following items were observed laying on top of the cubbies holding baskets in the large front room: four backpacks, seven blankets and one sheet. Owner confirmed the children's belongings did not all fit into the baskets provided for each child.

Description: Based on review and interview, facility did not ensure 2 of 6 staff records have an Office of Background Investigations (OBI) fingerprinting determination letter prior to a staff beginning employment. Evidence: The following was found during staff record review: 1. The record for staff #1 (start date of employment January 21, 2019) had a fingerprinting determination letter dated January 22, 2019. 2. The record for staff #2 (reported start date of employment January 21, 2019) had a fingerprinting determination letter dated February 6, 2019. 3. Interview with owner confirmed staff worked prior to receipt of the determination letter.
