Comments:
An unannounced monitoring inspection was conducted between 9:00am- 1:00pm. Upon arrival there were 89 children in care. A sample of 7 children and 1 staff record was reviewed in addition to 1 medication. Children were observed during lunch. Infants were observed sleeping in cribs, being fed and held by staff. One infant was sleeping in a swing.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure areas and equipment of the center were maintained in a safe condition. Evidence: 1. In the Kiva there were 4 wooden locker cubbies measuring about 4ft tall each. None of the lockers were secured.

Plan of Correction: The following corrective response was provided by the program director during the exit interview: The program director will request maintenance to secure all lockers to the wall.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center did not ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the unlocked cabinet above the children's cubbies in the 12-18 month room there was a bottle of Rustoleum.

Plan of Correction: The following corrective response was provided by the program director during the exit interview: The program director removed the chemical immediately

Standard #: 22VAC40-185-560-M
Description: Based on observation the center did not ensure staff sat with children during meals. Evidence: The preschool class was observed eating lunch. There was not a staff member sitting with the children during this meal. One staff was observed serving food and the other staff was observed putting down cots for nap.

Plan of Correction: The following corrective response was provided by the program director during the exit interview: There was a substitute in the classroom, however the program director will talk with all staff about sitting with the children during meals. The director will monitor for compliance.
