Comments:
An unannounced Monitoring Inspection was conducted on July 25 2019 from approximately 11:55 am until 3:00 pm. Upon entrance there were 81 children and 10 staff present with children. Additional staff were on site for administrative purposes. The record review sample consisted of 6 children's records and 5 staff records. Children were observed during their nap time and preparing for and eating snack. The infants were observed napping, freely playing and eating. There were no medication on site. Areas of noncompliance are noted on the violation notice. The results of the inspection were reviewed and verified with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure the areas of equipment inside and outside of the center were maintained in a safe and operable condition.

Evidence: On July 25, on the playground there was a wooden bench, (photographs taken exhibit C, D and E) with areas of chipped and peeling paint.

Plan of Correction: Bench sanded and repainted.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation, record review and staff interviews, the licensee did not ensure for children from birth to the age of 16 months there was one staff member for every four children.

Evidence: On July 25, 2019, between 11:35 am and 12:00 pm in the Mirrors 1 room, there were 6 infants with only one staff member, staff #1 . Center Director and staff #1 confirmed staff #2 went on break at approximately 11:35 am until 12:00 pm. Staff #2 returned to the room at approximately 12:00 pm and then there were two staff with 6 infants. The youngest child, child #1 in the Mirrors 1 room was 4 months old. The child to staff ratio was inadequate from approximately 11:35 am until 12:00 pm as a ratio of one staff to 4 infants was required.

Plan of Correction: Ratio will be maintained in classroom.

Standard #: 22VAC40-185-440-E
Description: Based on observation and staff interviews, the licensee did not ensure there were 12 inches between occupied cots.

Evidence: On July 25, 2019, during rest period in the Ladders 1 room there were between 7 and 8 inches between two occupied cots, cot for child #4 and child #5.

Plan of Correction: Cots adjusted to maintain 12 inches apart.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the licensee did not ensure a nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing.

Evidence: On July 25, the diaper pad, (photographs taken exhibit A and B ) in the Mirrors 2 room had 8 small rips that measured between 1/8 to 1/2 inch long. The diaper pad was no longer non-absorbent.

Plan of Correction: Pad repalced.

Standard #: 22VAC40-185-550-D
Description: Based on documentation review and staff interviews, the licensee did not ensure the center shall implement a monthly practice evacuation drill and a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.

Evidence: Center director, confirmed there was not documentation of a fire evacuation drill conducted in June 2019.

Plan of Correction: Drills updated monthly.
