Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the allowable variance process. An allowable variance form was left on site today with the Director.

Comments:
The following inspection took place between the hours of 10:30 am through 1:30 pm. Today, I met with the Staff of Villa Montessori Preschool located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 52 children with 10 staff; good ratios in all 4 rooms. A total of 5 children?s files and 5 staff files were reviewed. The children were engaged in the following: center activities within their classroom, bathroom breaks, hand washing, lunch, nap time, music, large motor skills outside on center playground, and other organized activities. Violations:
Standard #: 22VAC40-185-500-B
Description: Based on observation and interview, a nonabsorbent surface for diapering or changing was not being used for children younger than three years. Evidence - 1. On the date of inspection (07/23/2019), within the Toddler, a changing table according to staff has been converted into a sensory table. 2. A staff person in the toddler room stated (during an interview conducted on the date of inspection) that there were currently 10 children under the age of three years who were still in diapers. She also stated that these children were changed standing up. 3. The Director stated (during an interview conducted on the date of inspection) that this method of changing a child standing fall in line with the Montessori Philosophy and that is how staff are instructed and trained to change these children. The Director also stated that the sensory table was indeed a changing table, but is generally not used.

Plan of Correction: The sensory table will be converted back into a diapering table or an Allowable Variance (AV) shall be submitted in order for the center to continue to utilize and operate under a Montessori Philosophically based program.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on review, 1 of 5 staff files reviewed indicated that an authorize the child day center did not obtain a copy of the results of a search of the central registry (CPS) maintained pursuant to ? 63.2-1515 (and any child abuse and neglect registry or equivalent registry maintained) by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him or her. Evidence - 1. On the date of inspection (07/23/2019), an out-of-state CPS check for Washington, D.C. was not available for review for Staff #5. 2. Based on information provided on a CPS form contained on file, Staff #5 stated that residence was maintain in D.C. up until 06/01/2017. Staff #5 also stated on a completed Sworn Statement dated - 04/01/2019 that residence was maintain in D.C. within the past five (5) years. 3. The Director stated (during an interview conducted on the date of inspection) that a CPS check for Washington, D.C. in order to obtain information on Staff #5 was yet to be sent. 4. The start date for Staff #5 was listed as June 3, 2019.

Plan of Correction: The correct form in order to obtain this information for Staff #5 was printed out. This form shall be completed and sent out for results within 10 days. Proof that this documentation was sent shall be forwarded to the Licensing Inspector. Once documentation has been obtained, it shall be review with the Division of Licensing and placed on file at the center.
