Technical Assistance:
The following standards were discussed with the center director during the inspection: 22 VAC 40-185-290 (electrical outlets) and 22 VAC 40-185-330B (resilient surfacing).

Comments:
An unannounced monitoring inspection was conducted on 7/31/19 from 11:35am until 2:00pm. At the time of entrance, there were 31 children in care with 5 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed eating lunch, swimming in the outdoor pool, participating in arts and crafts and engaging in free play. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Three violations in the areas of administration and background checks are identified on the violation notice and were discussed with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview, the center did not ensure that they obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1. Child 1 has a date of enrollment of 6/17/19 and documentation of immunizations were not obtained by the center until 7/22/19. 2. The center director confirmed that documentation of immunizations for child 1 were not obtained by the child's first day of enrollment.

Plan of Correction: Going forward, the center will ensure documentation of up to date immunizations are obtained by the child's first day of attendance.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance. Evidence: 1. The record for child 1 is lacking documentation of a physical examination. Child 1 has a date of enrollment of 6/17/19 and was in care on the day of the inspection. 2. The center director confirmed that the record for child 1 does not contain documentation of a physical examination.

Plan of Correction: Child 1 has an appointment for a physical next week. The center will obtain a copy of the physical, which will be placed in the child's record. Going forward, the center will ensure each child's record contains documentation of a physical examination within the required time frames.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and interview, the center did not ensure that they obtain the results of the fingerprint based background check prior to the first day of employment. Evidence: 1. Staff 1 has a hire date of 6/3/19. The results of the fingerprint based background check for staff 1 was not received until 6/7/19. 2. The center director confirmed that staff 1 began employment prior to the results of the fingerprint background checks being received.

Plan of Correction: Staff 1 was hired prior to the new director beginning employment. Going forward, the center director will ensure all fingerprint based background checks are obtained before new staff begin employment.
