Comments:
The Licensing Inspector conducted an unannounced inspection on 7/12/19 in response to a complaint that was received in the Licensing office on 6/6/19 concerning infant feeding. Observations were conducted, staff were interviewed, and documentation was reviewed during the investigation. Based on the information gathered during the investigation, the evidence supports the complaint and is deemed to be valid. The areas of non-compliance associated with the complaint is on the attached violation notice.

Complaint related: Yes
Description: Based on observation, a review of five staff records and interviews, it was determined that the licensee did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Evidence: 1. Staff #1 and staff #2 were working together in the Infant classroom on 6/4/19. 2. The records for staff #1 and staff #2 were reviewed during the inspection on 7/12/19, and neither record contained documentation to demonstrate that either of the staff were Program Leader qualified. Staff #5 was told about the items missing from the staff records during the inspection. 3. Staff #4 (Program Director) who was not present during the inspection, but was spoken to on the telephone during the inspection, and was asked by e-mail on 7/18/19 to review the records for the staff #1and staff #2 that worked in the Infant classroom on 6/4/19, and provide documentation to demonstrate that either of the staff were Program Leader qualified. 3. As of 7/23/19, no documentation was provided to demonstrate either staff was Program Leader qualified.

Plan of Correction: The facility responded: Both staff members have completed the initial 24 hrs of training and staff 2 has over 6 months experience. Staff 1 was an assistant in the room. all documentation has been provided to you prior to this report being filled out.

Complaint related: Yes
Description: Based on a review of documentation and interviews, it was determined that the licensee did not ensure that the record of each child on formula shall contain the brand of the formula and the child's feeding schedule. Evidence: 1. The record for child #1 did not include the name of the child's formula or a feeding schedule. 2. Staff in the Infant classroom on 7/12/19 confirmed that currently there was not a feeding schedule for each child who is on formula. In addition, there was not a list of the brand of each child's formula.

Plan of Correction: The facility responded: A list of infant/formula was added to the room per your request. each child on formula is fed every 2-3 hours based off of the last feeding from home. Example if parent reports last feeding at 5am, then they will receive a bottle at 7. if the parent states they just ate at 7:30 before drop off, the next bottle is offered at 9:30am.

Description: Based on a review of documentation and interviews, it was determined that the licensee did not ensure that infants shall be fed on demand or in accordance with parental instructions. Evidence: 1. On June 4, 2019, the parent of child #1 (five months) brought in 4 bottles for her child to to be fed during the day. 2. The parent arrived to pick up child #1 at 5:00pm on 6/14/19, and she said "she noticed that his face look sunken and his eyes were red. Upon reviewing his daily chart, she noticed that child #1 was given his first bottle at 8:00am and his last bottle was given at 11:00am. 3. When the parent of child #1 questioned staff #3 if maybe child had been given another bottle since 11:00am, but it just wasn't recorded, staff #3 said she hadn't been in the Infant classroom all day, and only came in around 3:30pm. 4. Staff #3 checked the refrigerator and found that child #1 still had two full unused bottles, which confirmed that he had not been given a bottle since 11:00am. 5. The parent of child #1 stated that she sends 4 bottles with the assumption he would drink three bottles and have the fourth bottle as back up.

Plan of Correction: The facility responded: Infants are fed on parental instructions. An infant feeding list with parent signatures will be added to the infant room. Staff were all retrained on infant feeding and documentation. a digital daily sheet was implemented so that management can see all recorded info.
