Comments:
A Subsidy Health and Safety inspection was conducted on 08/01/2019 to review supplemental health and safety requirements. Licensing Inspector conducted a Subsidy Health and Safety Inspection between 9:00am and 12:30pm. At the time of the tour there were 22 children in care with 5 staff present. A sample of 1 child and 3 staff records were reviewed. Children were observed playing in centers, watching Elmo and during lunch. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-580-E-1
Description: Based in a record review the vendor did not ensure all staff had a current certification in cardiopulmonary resuscitation (CPR). Evidence: Staff #1 (hired on 03/26/19) did not have documentation of having a current certification in CPR.

Plan of Correction: The following corrective action was provided by the Assistant Director during the exit interview: Staff #1 has the certification, however it was never printed and placed in the file. Staff #1's CPR certification will be obtained and placed in the file.

Standard #: 22VAC40-665-580-E-2
Description: Based in a record review the vendor did not ensure all staff had a current certification in First Aid. Evidence: Staff #1 (hired on 03/26/19) did not have documentation of having a current certification in first aid.

Plan of Correction: The following corrective action was provided by the Assistant Director during the exit interview: Staff #1 has the certification, however it was never printed and placed in the file. Staff #1's first aid certification will be obtained and placed in the file.

Standard #: 22VAC40-665-770-B
Description: Based on a record review and interview, the vendor did not ensure all required components were included in the emergency plan. Evidence: The emergency plan did not include procedures to address reuniting children with a parent or an authorized person designated by the parent to pick up the child during a shelter-in-place or intruder scenario. The Assistant Director could not locate the procedure and called the owner. The owner confirmed to the inspector this procedure was not complete and not in the plan.

Plan of Correction: The following corrective action was provided by the Owner (via telephone) and Assistant Director during the exit interview: The reunification plan will be more detailed and include procedures for a shelter-in-place and an intruder scenario.

Standard #: 22VAC40-665-770-B-5
Description: Based on a record review and interview the vendor did not ensure there was procedure in the emergency plan that addressed the continuity of operations procedures to ensure that essential functions are maintained during an emergency. Evidence: The emergency plan did not include a procedure addressing the continuity of operations procedures to ensure that essential functions are maintained during an emergency. The Assistant Director could not locate the procedure and called the owner. The owner confirmed to the inspector this procedure was not complete and not in the plan.

Plan of Correction: The following corrective action was provided by the Owner (via telephone) and Assistant Director during the exit interview: The vendor is not exactly sure how to create this procedure and will seek outside help on developing this.
