Comments:
The Licensing Inspector conducted an unannounced complaint inspection ifrom 7:25 am -10:30 am in response to complaints that were received by the licensing office on July 30, 2019 and July 31, 2019. Interviews were conducted with staff relating to the allegations of classrooms being out of ratio. Staff attendance sign in forms, children attendance forms and children diapering schedules for July 29, 2019, July 30, 2019 and July 31, 2019 were reviewed. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. The violation notice was reviewed with the Infant teacher who is also acting as director until the new center Director starts on August 19th. Complaint related: Yes
Description: Based on children attendance forms, staff attendance records and staff interviews, the center is not maintaining ratio of 1 staff for every 4 children from birth to the age of 16 months. Evidence: The children attendance records, and staff sign in sheets were reviewed for Thursday, August 1, 2019. At 10:00 am the Licensing Inspector observed 9 children, ages 12 months to 16 months with 2 staff. According to staff interviews no other staff will report to this classroom today.

Plan of Correction: The new center director starts August 19th. I will call management to ask for more staff.

Description: Based on children attendance forms, staff attendance records and staff interviews, the center is not maintaining a ratio of 1 staff for every 5 children for children 16 months old to two years. Evidence: The children attendance records, and staff sign in sheets were reviewed for Tuesday, July 30, 2019. According to staff interviews from 9:00 am until 12:00 pm there were 2 teachers with 14 children, ages 16 months to two years old. The third teacher arrived from another center at 12:00 pm.

Plan of Correction: The new center director starts August 19th. I will call management to ask for more staff.

Description: Based on children attendance forms, children diapering schedule, staff attendance records and staff interviews, the center is not maintaining a ratio of 1 staff for every 8 children two years old. Evidence: The children attendance records, children diapering schedule and staff sign in sheets were reviewed for July 29, 30, and 31 2019. According to staff interviews and children records the class was out of ratio from the hours 9:30 am- 5:00 pm. On Monday, July 29th there were 17 children, July 30th there were 18 children and July 31st there were 17 children.

Plan of Correction: The new center director starts August 19th. I will call management to ask for more staff.

Description: Based on children attendance forms, staff attendance records and staff interviews, the center is not maintaining a ratio of 1 staff for every 10 children three years to the age of eligibility to attend public school. Evidence: The children attendance records and staff sign in sheets were reviewed for July 29, 30, and 31, 2019. According to staff interviews and children attendance records the class was out of ratio from the hours 10:30 am- 4:30 pm. Each day there were 11 children with one staff.

Plan of Correction: The new center director starts August 19th. I will call management to ask for more staff.
