Standards related to food service, (USDA required serving size and menus) were reviewed.

An Unannounced Monitoring Inspection was conducted on August 1, 2019 from approximately 9:55 am until 1:20 pm. Upon entrance there were 52 children and 9 staff present with children. Additional staff were on site for administrative and program purposes. The record review sample consisted of 6 children's records and 5 staff records. Children were observed playing outside, eating lunch and during story time. Infants were observed freely playing, eating and during the diaper changing process.

Standard #: 22VAC40-185-330-B Description: Based on observation and staff interviews, the licensee did not ensure that were playground equipment is provided, the resilient surfacing shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use. Falls zones shall not include barriers for resilient surfacing.
Evidence: On the older children's playground, the resilient surfacing is inadequate around the base of the green slide chute, as it measures between 3- 4 inches and around the metal parallel bars, as it measures between 4-5 inches. A minimum of 6 inches of resilient surfacing is required for the green slide and the parallel bars.Plan of Correction: The owner will either till it or add more mulch.
Standard #: 22VAC40-185-560-F Description: Based on observation and staff interviews, the licensee did not ensure that when the centers choose to provide meals or snacks, the center shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence: In the Pre-K room, twenty children, ages 3-5 were observed during lunch and they were served only 1/2 cup of milk and USDA requires they are served at least 3/4 cup of milk. The center staff verified the measurement of 1/2 cup.Plan of Correction: They will make the initial serving consist of 3/4 cup of milk. Center Director will purchase new cups to meet the measurement of milk required.
