Comments:
An unannounced monitoring inspection was conducted with center director and center staff to determine compliance with the licensing standards. There were 34 children present, ranging in age from two years to 12 years. The children were waking from rest time, returning from a field trip to the bounce house, and playing with age appropriate toys. Three staff files and four children's files were reviewed - please see Supplemental Summary for details. Three medications for three children were reviewed - two staff present have E-MAT training. Evacuation drills have been conducted monthly; the last shelter in place drill was conducted 3/11/2019. The last fire inspection was conducted 2/27/2019. Description: Based on observation, the facility failed to lock hazardous substances such as cleaning materials. Evidence: in the school age building bathroom, there were unlocked cleaning supplies including bleach, Great Value bleach solution, and Glade aerosol spray, under the sink

Plan of Correction: Staff will be reminded to lock all cleaning supplies.

Standard #: 22VAC40-185-510-N
Description: Based on review of medications and staff interview, the facility failed to return a medication back to the parent within 14 days of the expiration of the medication. Evidence: a medication for Child F that expired 10/2018 was still in the medicine box.

Plan of Correction: Center director returned the expired medicine to the parent during today's inspection.
