Comments:
The Licensing Inspector conducted an unannounced complaint inspection with the local agency on 7/11/19 in response to a complaint that was received in the Licensing office on 7/1/19 concerning supervision and care of the children. Observations were conducted, staff were interviewed, and documentation was reviewed during the investigation. Based on the information gathered during the investigation, at this time the evidence does not support the complaint and is deemed to be not valid. However, there were violations that resulted from this inspection. The area of non-compliance associated with the complaint is on the attached violation notice.

Complaint related: No
Description: Based on observation, a review of four staff records and interviews, it was determined that the licensee did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Evidence: 1. Based on information gathered during interviews with staff, it was determined that staff #4 was working in the Infant classroom with 4 children alone on 6/27/19. 2. The record for the staff #4 did not contain documentation to demonstrate that she was Program Leader qualified. 3. Staff #5 (Program Director) reviewed the record for the staff #4, and confirmed that staff #4 was not Program Leader qualified as she is still in high school.

Plan of Correction: The facility responded: Staff received training regarding ratio and the roles and responsibilities of Lead Teachers and Teacher Assistants. The center will ensure a staff member, who is Lead Teacher qualified, will be present in each classroom.

Complaint related: No
Description: Based on a review of four staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #1 (date of hire 4/22/19) did not contain documentation of a completed search of the central registry finding. 2. Staff #5 (Program Director) reviewed the record for the staff #1, and said that the search of the central registry finding has not been received. 3. On 7/12/19, staff #5 forwarded by e-mail the completed search of the Central Registry for staff #1. However, the date of completion was 6/30/19, which is more than 30 days after staff #1 began employment.

Plan of Correction: The facility responded: The central registry check was sent to licensing via email on 7/12/19 when it was received from the state central registry office. To prevent a delay in the future, the Director will contact the central registry office if the employee results have not been received within at least 15-20 days.
