Standard #: 22VAC40-185-330-B Description: Based on a tour of the playground the facility failed to ensure that resilient surface material depth complies with minimum safety standards. Evidence: 1. There is 3-4 inches of resilient surface material at the base of the slides. Plan of Correction: The administrator will ensure that all areas which are required to have resilient surface material are maintained to meet the 6 inch requirements.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on the review of seven staff files, the facility failed to ensure that all background checks are conducted on all employees. Evidence: 1. Staff #3 reported having lived in two states in the last five years. There is no central registry clearance on file for one of those states. Plan of Correction: The administrator will ensure that all required background checks are conducted on all staff.
