Technical Assistance:
An exit interview was conducted with the director to discuss the result of today's inspection.

Comments:
An unannounced monitoring inspection was conducted today, July 25, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 3:30pm and concluded at 5:45pm. Upon arrival, the following ratios were observed: Infant room: 7 infants/ 2 staff Toddler room: 7 toddlers/ 1 staff Preschool room #1 (3yrs-4yrs): 5 children/ 1 staff Preschool room #2 (3yrs-4yrs): 10 children/ 2 staff School age room: 26 children/3 teachers Children were observed interacting with staff, playing with toys, playing dodgeball, and participating in arts & crafts. Today's snack consisted of pretzel and watermelon. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All classrooms were inspected to include main hallways, children's restrooms, and the outdoor blacktop area used for outdoor play. Parent pickup procedures were observed during this inspection. The center had the required postings displayed upon entering the center and in children's classrooms. The center's first aid kit and emergency supplies were inspected with additional supplies found. Five children's records and five staff records were inspected with the required information present. The following was also inspected: daily attendance, annual fire & health inspections, fire & shelter-in-place drills, parent agreements, and other pertinent information. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on the inspection of administrative records, the required references were not obtained. Evidence: During the inspection of five staff records, it was discovered staff #3 who was rehired in May 2018 did not have two references in record. Licensing standards require two or more references to be checked before employment or volunteering, with documentation maintained to review.

Plan of Correction: Per director: I will obtain and contact the two references given and update the record.

Standard #: 22VAC40-185-90--A
Description: Based on the inspection of administrative records, the required parent agreement statements were not in the record and signed by the parent. Evidence: During the inspection of five children records, it was discovered the center did not have in the parent agreement and signed by the parents the following: 1. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; and 2. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

Plan of Correction: Per director: I will add these statements to the parent agreement and have parents to sign the form.
