Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the capacity provided by the County of Loudoun. The center capacity will be to be adjusted from 250 down to 242 in order to reflect the accurate number. Discussion of the orientation documentation. The model form was provided by the Licensing Inspector (LI) and was recommended for future use. Discussion was also held with regards of posting of each child's official start date on the Proof of Identification page.

Comments:
The following inspection took place between the hours of 8 am through 2 pm. Today, I met with the Staff of the Montessori Academy at Belmont Greene located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 20 children with 8 classroom staff; good ratios in all 4 rooms. A total of 5 children?s files and 10 staff files were reviewed. The children were engaged in the following: drop-off, center activities within each classroom, breakfast, bathroom breaks, hand washing, diapering, games, large motor skilled activities, lunch, after-noon pick up, and other organized activities. Standard #: 22VAC40-185-160-C
Description: Based on review, 1 of 10 staff files reviewed indicated that it has been more that two years since a staff person has obtained updated tuberculosis (TB) documentation. Evidence - 1. On the date of inspection (08/02/2019), updated TB documentation was not available for Staff #1. 2. The most current TB documentation on file was dated - 01/24/2017.

Plan of Correction: A test was administered for Staff #1 two days ago. A reading has been scheduled. Once the documentation has been obtained it shall be placed on file.

Standard #: 22VAC40-185-70-A
Description: Based on review, 2 of 10 staff files reviewed indicated that not all of the required documentation was contained within each staff's record. Evidence - 1. On the date of inspection (08/02/2019), education documentation was not available for Staff #2, and Staff #4. 2. It was believed that this documentation was on file at one time, as indicated by the check list kept with each file, but was not available at time of inspection.

Plan of Correction: Staff will provide an addition copy of this information to be placed on file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, two areas within the center were not maintained in a safe condition. Evidence - 1. On the date of inspection (08/02/2019), a light was not working within the bathroom on the left side (without the door) within the Toddler Room. 2. A blind cord was observed hanging down to the floor; accessible to the five (5) children in care within the Toddler room.

Plan of Correction: 1. Light bulb shall be switched out. 2. Corrected on the date of inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, cleaning materials were not kept in a locked place using a safe locking method that prevents access by children. Evidence - 1. On the date of inspection (08/02/2019), cleaning supplies were observed in an unlocked area within the following rooms: Primary 1 (on an upper shelf within the bathroom), Primary 3 (within an unlocked closet in the back of the room), and the Toddler room (within a cabinet). 2. Staff moved these items to a locked area.

Plan of Correction: Corrected on the date of inspection.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on review and interview, 1 of 10 staff files reviewed indicated that the Child Day Center (CDC) did not obtain a copy of the results of a search of the central registry (CPS check) maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him or her. Evidence - 1. On the date of inspection (08/02/2019), an Out-of-State CPS check from the State of West Virginia was not available for review for Staff #9. 2. The Sworn Statement dated - 10/01/2018, completed by Staff #9 states indicates residency in West Virginia. 3. Staff #9 stated (during an interview conducted on the date of inspection) that while a Virginia address is used for mail, Staff #9's residence is outside the Commonwealth of Virginia.

Plan of Correction: An Out-of-State CPS check shall be obtained for Staff #9.
