Comments:
An unannounced Subsidy Health and Safety inspection was conducted on Thursday, July 25, 2019 to determine the center's compliance with Subsidy Health and Safety requirements. The inspector arrived at 9:40 am and concluded the inspection at approximately 12:15 pm. The census for today?s inspection consisted of 17 children in the direct care of 3 staff. The center's owners assisted the inspector throughout the inspection. Upon arrival, the children were engaged in various activities. The children in the Pre-K classroom were observed during a group coloring activity and playing in different centers. The children in the School Age classroom were observed coloring, reading books, and interacting with staff and their peers. Both groups were later observed during a scheduled special center activity. Later, the children were observed taking bathroom breaks, and washing hands. The center has age and stage appropriate equipment and the classrooms are decorated with the children's artwork. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. Transportation is not provided and medication is not administered at this time. The most recent health inspection was on 05/23/2019. The most recent fire inspection was on 06/07/2019. Five children's records and five staffs' records were reviewed during today's inspection.

A licensing inspection was also conducted on this date; the two inspections combined cover all Subsidy Health and Safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: 22VAC40-665-650-E
Description: Based on a review of the center's policies and interview on 07/25/2019, it was determined that the vendor did not ensure that they shall develop and implement a written policy and procedure that describes how the vendor will ensure that each group of children receives care by consistent staff or team of staff members. Evidence: 1) There was no written policy and procedure regarding consistent staff in each group of children available for viewing during the inspection. 2) During interview, a member of management confirmed that a written policy and procedure regarding consistent staff in each group of children had not been developed in writing.

Plan of Correction: The vendor will be adding the written policy by Monday.

Standard #: 22VAC40-665-770-B-5
Description: Based on a review of documentation and interview on 07/25/2019, the vendor did not ensure the emergency preparedness plan contained continuity of operations procedures to ensure the essential functions are maintained during an emergency.

Evidence: 1) During the inspection, the licensing inspector reviewed the emergency preparedness plan. The plan did not contain a written plan on how the center will ensure continuous operation procedures are in place to ensure essential functions are maintained during an emergency. 2) During interview, a member of management reported the information had not been added to the emergency preparedness plan to date.

Plan of Correction: The vendor will be adding policies to the emergency plan by Monday.
