Comments:
An unannounced monitoring inspection was conducted between 9:00am and 11:30am. Upon arrival there were 79 children in care with 14 staff present. A sample of 4 staff and 7 children's records were reviewed, in addition to 3 medications. Children were observed diapering/toileting, playing in learning centers and during an art project. Infants were observed on the floor with staff and sleeping in cribs.

Violations:
Standard #: 22VAC40-185-500-A
Description: Based on observation the center did not ensure all requirements were met when hand washing. Evidence: 1. A staff member in the infant room was observed during a diaper change. After changing the soiled diaper, the staff member did not wash her hands after removing gloves from her hands. 2. A staff member in the twos room was observed wearing gloves while assisting children in the bathroom. The staff member was observed removing gloves from her hands between diaper changes, however she did not was her hands.

Plan of Correction: All staff members were retrained on 7/22/19 on proper handwashing procedures. Safety captain and management will periodically inspect to ensure compliance.

Standard #: 22VAC40-185-500-B
Description: Based on observation the center did not ensure all requirements were met when diapering. Evidence A staff member did not use a foot operated trash can to dispose of a soiled diaper. In the infant room a staff member was observed changing a child's diaper. After the change, the staff member discarded the soiled diaper, wipes and paper changing pad by using her hands to open the lid of the trash can.

Plan of Correction: All staff members were retrained on 7/22/19 on proper diaper changing procedures. Safety captain and management will periodically inspect to ensure compliance.

Standard #: 22VAC40-185-540-C
Description: Based on observation the center did not ensure all required first aid supplies were included in all of their first aid kits. Evidence: The center's main first aid/emergency kit did not contain a thermometer.

Plan of Correction: During the inspection this was corrected. Teachers will notify management within 24 hours of anything missing from the bags. The safety captain inspects all bags monthly to ensure compliance and will report any deficiencies to management within 24 hours. Management will periodically check to ensure compliance.

Standard #: 22VAC40-191-60-C-2
Description: Based on a record review the center did not ensure all staff had the required background checks. Evidence: Staff #4, hired 03/20/19, did not have a a central registry check with the center. The staff's record had the results of a central registry check, however it was not from entity in which she was is employed.

Plan of Correction: Going forward all new hires and any transfers must have all paperwork complete and sent out on Day 1 in order to continue employment. Follow up will be done and documented before the 30 day time limit. Management will utilize outlook to maintain current records timely and will do periodic inspections to ensure compliance.
