Comments:
An unannounced renewal inspection was conducted beginning at 11:45 a.m. and ending at 3:30 p.m. There were 54 children in care ranging in age from 10 months to 11 years under the care and supervision of 9 staff. Infants were observed sleeping. The Preschool I class was observed having lunch. Lunch today was turkey ham sandwiches with cheese, green beans, pears and milk. Two year olds were observed during 'shake, wiggle and grow' time. Toddlers, Preschool II and PreK were observed preparing for nap. Schoolagers were observed during quiet reading time. The outdoor play area and playground equipment was inspected. Eight children's records and six staff records were reviewed. Staff training hours were reviewed. There are12 Medication Administration Training (MAT) certified staff however, there are currently no medications on site. Emergency drills were reviewed and are up to date. Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in safe condition. Evidence: In the Preschool I classroom, in the bathroom the bolt that connects the toilet to the floor is exposed with at least eight thread lengths and creates impalement and scratching hazards. In the Preschool II classroom, in the bathroom the bolt that connects the toilet to the floor is exposed with at least six thread lengths and creates impalement and scratching hazards.

Plan of Correction: Maintenance was contacted during the inspection and a work order placed.

Standard #: 22VAC40-185-500-A
Description: Based on observation, staff failed to ensure children's hands were washed with soap and running water or disposable wipes after eating meals. Evidence: The Preschool I classroom was observed finishing lunch. After eating, children threw away remaining food and drink and put their chairs away and then went to the carpet for quiet activity.

Plan of Correction: The staff member will be retrained on handwashing.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of documentation, the center failed to request out of state central registry background check for all states that an employee has resided in in the past five years. Evidence: Staff #2, indicated out of state residence in South Carolina on the sworn statement dated 2/23/2019. Staff #2's resume indicated that there was also out of state residence in Georgia. This out of state residence was not indicated on the sworn statement and there was no Georgia central registry background check. Staff #5 indicated out of state residence in Connecticut on the sworn statement dated 8/28/2017 and there was no documentation of a Connecticut central registry background check request.

Plan of Correction: Staff #2 was immediately asked to update the sworn statement and both Staff #2 and Staff #5 completed the central registry background check requests during the inspection and they have been requested electronically and will be mailed today.
