Comments:
An unannounced renewal inspection was completed on 8/5/2019. There were 78 children present in six groupings (infant, toddlers, two's, preschoolers, older preschoolers and school age). Eight children's records and Nine staff's records were reviewed. The inspector arrived at the center at 10:00 AM and departed at 2:00 PM. The children were observed during arrival, during free play time in the classrooms, outside time, games, during table toy play time , during story times, music during lunch, and during nap. There was discussion with the owner about the following: ensuring all medications has all necessary boxes for child # 7, positioning staff during nap time to ensure all children can be seen, making sure that persons not authorized to pick up is completed on each child's enrollment form and ensure that outside time occurs daily.

Violations:
Standard #: 22VAC40-185-150-A
Description: Based on review of eight children's records, the center failed to ensure that all immunizations were signed and dated. Evidence: 1. Child # 4 did not have physician's signature on his immunization record.

Plan of Correction: The director will have the parent to obtain a signature from the doctor's office.

Standard #: 22VAC40-185-450-D
Description: Based on observation, the center failed to ensure that all pillows and items used as pillows were covered with a pillow case. Evidence: 1. Observed in the blue room, two children using items as pillows and they were not covered with a pillow case. One child had a small pink fuzzy pillow and the other child had a large stuffed duck that was used as a pillow that did not have a cover.

Plan of Correction: The director will ensure that all pillows and items used as a pillow will have a case.

Standard #: 22VAC40-185-500-A
Description: Based on observation of diapering, the center failed to ensure that all hands were washed as required by the standard. Evidence: 1. Observed in the infant room, staff # 9 change three infant/toddlers and did not wash her hands in between each diaper change.

Plan of Correction: The director will go over the directions for hand washing and will monitor.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interviews with staff, the center failed to ensure that examination gloves were used during diaper changes. Evidence: 1. Observed staff # 9 using food handlers gloves during a diaper change. Per the requirements of the standard examination gloves are required. 2. The staff person stated that they use this type of glove for wet diapers only.

Plan of Correction: The food handlers gloves were removed from the diapering area during the inspection by the infant staff.
