Comments:
An announced initial inspection was conducted on 08/06/2019 from 9:15 A.M.-12:00 P.M. The facility was toured and the outdoor play area and classrooms were measured for capacity. Some items observed/reviewed during the inspection included: required posted information, parent drop off/pick-up procedures, emergency maps and procedures, first -aid kit, emergency supplies, two staff records, injury prevention log, and emergency plan. A recommendation will be made for an initial six-month conditional Child Day Center license with a capacity of 32 children and an age range of 21/2 years through five years. Description: Based on review of evacuation/shelter-in place maps, the center failed to ensure that all maps contained the required information. Evidence: 1. The maps in Classroom A and Classroom B did not indicate the shelter-in place location. 2. Staff verified that the maps did not indicate the shelter-in-place location.

Plan of Correction: Administration will add the shelter-in-place location on each map.
