An unannounced monitoring visit and facility tour was conducted from 11:45 am- 1:11 pm. At the time of inspectors' arrival there were 39 infant to school aged children in care with 10 staff members. Children were observed participating in indoor play, lunch, nap, transitioning between activities, and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports were also reviewed. Assistant Director reported that there are no medications being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-185-130-B Description: Based on record review it was determined the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #2(enrolled 12/10/18) did not contain updated immunization documentation every six months. The immunization record in the record was dated 1/21/19, at which time child #2 was under two years old. 2. The record for child 3(enrolled 8/21/17) did not contain updated immunization documentation every six months. The immunization record in the record was dated 12/15/17, at which time child #3 was under two years old. Plan of Correction: -
Standard #: 22VAC40-185-140-A Description: Based on record review it was determined the center did not ensure each child had a physical by or under the direction of a physician within the required time frame. Evidence: The record for child #1(enrolled 4/6/19) did not contain documentation of a physical exam. Child #1 was in care during this inspection. Plan of Correction: -
Standard #: 22VAC40-185-270-A Description: Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: In the 4 year old and school age sections of the building there was broken plaster, peeling paint, and splintered wood veneer on the kitchen play set. Plan of Correction: -
