Comments:
An unannounced monitoring inspection was conducted from 12:30pm-3:00pm. There were 104 children in the direct care and supervision of 18 staff. A sample size of 10 children's records, 4 staff records, evacuation drills, the first aid kit, emergency materials, 2 emergency medications, 3 diaper ointments, the playground, and the physical plant were reviewed. The children were observed reading books, taking a nap, and in the infant rooms teachers were blowing bubbles for the infants. Violations:
Standard #: 22VAC40-185-450-A
Description: Based on observation, 1 out of 17 Pre-K children did not have a bottom cover during the designated rest period. Evidence: 1. In the Pre-K classroom, 1 child was missing a bottom cover.

Plan of Correction: We will make sure that all children have a top and bottom cover during nap time.

Standard #: 22VAC40-185-520-A
Description: Based on observation, 2 out of 3 diaper ointments reviewed were expired. Evidence: 1. Child #11's diaper ointment expired on 3/2019. 2. Child #12's diaper ointment expired on 7/2019.

Plan of Correction: The diaper ointments were removed from the classroom during the inspection and will be given back to the parents.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of staff records, 1 out of 4 staff did not have a central registry finding within 30 days of employment. Evidence: 1. Staff member #2 (hire date: 9/17/2018) did not have documentation of a central registry finding on file.

Plan of Correction: We will send it out overnight and send it to you as soon as we receive it back.
