Comments:
An unannounced renewal inspection was conducted today from 11:00am-3:00pm. There were 125 children (infants-11yrs.) directly supervised by 50 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, singing in Chinese, enjoying beach week, coloring, listening to stories, being bottle fed, napping, and eating lunch. Today's lunch consisted of baked chicken with fresh herbs, summer pesto pasta and wheat bread with milk. Diapering and hand-washing procedures were also observed. Cribs were also inspected. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not obtain updated tuberculosis (TB) test or screening for each staff member every two years. Evidence: Staff#2's record contains a TB test/screening that was last updated on 8/1/2016. Staff#10's record contains a TB test/screening that was last updated on 8/18/2016.

Plan of Correction: We will have the staff take it back to their doctor's to update it.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center were not maintained in a safe and operable condition. Evidence: The boys' bathroom located on the third floor of the facility is missing approximately 5x5 inches of floor tile exposing the concrete in three different areas near the urinals and the toilet, creating a possible tripping hazard. The vent screen covering the A/C unit in the girls bathroom on the third floor is showing a small area on the lower right side of the panel that is rusted and warped.

Plan of Correction: An order has been placed and should be taken care of by the end of this week.

Description: Based on observation, the findings of the most recent inspection of the facility were not posted. Evidence: The Violation Notice posted was dated 8/8/2018, the Violation Notice issued on 4/4/2019 was not posted.
