Comments:
An unannounced monitoring inspection was conducted today from 9:00am-11:30am. There were 56 children supervised by 9 direct care staff. The physical plant, programming, 12 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, singing and making a fruit salad. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations:
Standard #: 22VAC40-185-260-A
Description: Based on review of emergency information, the has not been an annual fire inspection report completed. Evidence: The current fire inspection report expired on June 30, 2019.

Plan of Correction: I called the fire department and scheduled an inspection for August 15, 2019.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the outside of the center are not being maintained in a clean, safe and operable condition. Evidence: The top right playground fence, has a 2-3" gap towards the bottom of the fence, that is accessible to children and could cause a possible entrapment. There is a broken toy bin that is accessible to children. The stage area on the top right playground has wood that is chipped and splintering. The top lip of the retaining wall has a piece that lifts up and can cause a danger to the children. A decoration on the back fence is chipped, leaving a sharp edge exposed.

Plan of Correction: All maintance items have already been sent to our facility operations to have corrected. I expect them out a day this week.

Standard #: 22VAC40-185-550-M
Description: Based on review of records of injuries, not all records are complete. Evidence: Not all required information has been documented on records, items missing include time and date parent was notified, and two signatures.

Plan of Correction: I will review with all staff at our next meeting, which is scheduled for 8/14/19 and I will review the reports with management to ensure all required information is documented.
