Standard #: 22VAC40-665-540-A Description: Based on record review, the licensee did not ensure prior to coming in contact with children and the documentation has been completed within the last 30 calendar days of the date of employment communicable tuberculosis (TB)is determined to be free of communicable tuberculosis (TB). Evidence: Staff #1 hire date May 2, 2019 tuberculosis screening results were completed 02/28/2019 which was not completed within the last 30 calendar days of the date of employment. Plan of Correction: Management has been retrained on ensuring that all new hire TB tests are to be less than 30 days old upon hire. DM will review all new hire files monthly for compliance.
