Comments:
A Subsidy Health and Safety inspection was conducted on July 31 2019 between 12:00pm and 2:00pm to review supplemental health and safety requirements. There were 78 children in the direct care of 16 staff members. Children were observed reading, napping, and returning from a field trip. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on record review, 1 of 5 new staff records reviewed did not have documentation that the staff member is determined to be free of communicable tuberculosis (TB) within the last 30 calendar days of the date of employment. Evidence: Staff #2 record has a documented tuberculosis screening dated 8/18/2017.

Plan of Correction: Correction: Staff #2 had a negative TB test on file at the time of visit. Staff #2 obtained a new TB test to have on file to reflect a negative TB test. Test within last 30 days. Date of Correction: 7/31/2019
