Comments:
A Subsidy Health and Safety inspection was conducted on 8/6/2019 to review supplemental health and safety requirements. There were 35 children (infants-5yrs.) supervised by 13 direct care staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside on the playground, playing in the water table, having circle time, beading, and building with connectors and Legos. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the Violation Notice. Description: Based on record review, 1 of 3 staff and volunteer records did not contain documentation of tuberculosis screening submitted at the time of employment and prior to coming into contact with children. Evidence: Volunteer A's record did not contain documentation of TB results.

Plan of Correction: Going forward, all volunteers will have a negative TB screening on file before volunteering with the program.
