Technical Assistance:
A supplemental page was provided. As we discussed, your program will be using three classrooms plus the multi-purpose room for the care of children. You will also be using the grassy space for non-mobile infants and the playground for rest of the children. At this time no transportation will be provided by the center. Medications will be administered, as needed. The center will administer over-the-counter skin products. Food will be provided by the center.

Comments:
Thank you for your time and assistance during the announced initial licensing inspection conducted today, August 8, 2018. The on-site inspection was from 10:00 a.m. to 3:15 p.m. The facility and the outdoor play areas were toured on this date. Measurements were taken for four classrooms and the play areas were measured. At the time of the inspection, no children were in care. The records for two staff members were reviewed. Policies and procedures were reviewed for compliance, along with applicable licensing standards. Some items observed, reviewed, and discussed during the visit included: classroom set-up and arrangement, classroom equipment, first-aid kit, emergency supplies, toys and equipment, supplies, napping equipment and linen care, cribs, emergency preparedness policy and procedures, the play yard, required posted information, and warm water temperature. Compliance could not be determined for all of the standards. The exceptions to compliance were noted on the violation notice and the risk ratings assigned to the violations were noted on the supplemental page. A recommendation will be made for the issuance of a Conditional Child Day Center license with a capacity of 44 children and an age range of infants through twelve years. Violations:
Standard #: 22VAC40-185-240-D-5
Description: Based on review of two staff records and confirmation from the director, no staff member had obtained instruction in performing daily health observations of children. Evidence: 1. There was no documentation that either of the two staff members had current daily health observation training. 2. The director confirmed that her training had expired and the other staff member did not have the training.

Plan of Correction: The staff members will obtain daily health observation training before the center opens and the director will ensure that at least one staff member with the training will be present during all operating hours.

Standard #: 22VAC40-185-540-C
Description: Based on review of two first aid kits and discussion with the director, the first aid kits did not contain all of the required items. Evidence: 1. There were no triangular bandages in either of the first aid kits. 2. The director confirmed that she had forgotten to get the required two triangular bandages for the first aid kit.

Plan of Correction: The director will obtain the triangular bandages as soon as possible.

Standard #: 22VAC40-185-550-B
Description: Based on review of the center's emergency preparedness plan and verification from the director, the emergency preparedness plan did not contain all of the required components. Evidence: 1. The following required items were missing from the emergency preparedness plan: 24-hour contact information for the center's emergency officer and emergency back-up officer, notification of local authorities, staff training requirements, drill frequency, and when the plan would be reviewed and updated. 2. The director verified these components were not present in the plan.

Plan of Correction: The director will add all required items to the emergency preparedness plan and e-mail it to the licensing inspector.
