A complaint investigation was initiated on August 5, 2019 and ended on August 9, 2019 regarding playground safety, and center policies. Interviews were conducted, documentation was reviewed, as well as an inspection of the playground. There was not a preponderance of evidence to support the allegation regarding center policies; however, the complaint was deemed valid with regard to unsafe conditions on the center's playground. Standard #: 22VAC40-185-330-B Complaint related: Yes Description: Based on observation, the resilient material under the playground equipment and in fall zones does not meet the minimum standards of 6 inches. Evidence: Resilient material (mulch) is less than 1/4 inch under the slide exits, swings, and climbing equipment fall zones. This violation was previously cited on 4/16/19. Plan of Correction: Mulch was ordered on 8/6 and delivered on 8/7
