Comments:
An announced initial inspection was conducted on 8/7/19 from 10:35 am until 1:00 pm at Culpeper Baptist Child Development Center to review the standards for licensed child cay care centers. All standards were reviewed with the administration. The physical plant and playground were observed. Violations were found during this inspection and are documented on the violation notice. Violations:
Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure outlet protective covers were in every outlet.

Evidence:

None of the outlets in the cafeteria and gym had outlet covers.

Plan of Correction: Outlet covers were added to the gym and cafeteria the same day as the inspection.

Standard #: 22VAC40-191-40-C-1-A
Description: Based on interview, the center failed to obtain sworn statement or affirmation and search of central registry for the three applicants listed on the business entity page of the application. Evidence: The director stated they were unaware they needed to have the three applicants listed on the business entity page of the application complete sworn statement or affirmation and search of central registry.

Plan of Correction: The three applicants will be required to complete the paperwork for a sworn statement or affirmation and search of central registry within five days. The central registry documents will be mailed as soon as they are received.

Description: Based on interview, the center failed to obtain sworn statement or affirmation and search of central registry for the three agents listed on the business entity page of the application.

Evidence:

The director stated they were unaware they needed to have the three agents listed on the business entity page of the application complete sworn statement or affirmation and search of central registry.

Plan of Correction: The three agents will be required to complete the paperwork for a sworn statement or affirmation and search of central registry within five days. The central registry documents will be mailed as soon as they are received.

Description: Based on interview, the center failed to obtain fingerprint background checks for all applicants and agents.

Evidence:

The director stated they were unaware they needed to have the three applicants and three agents listed on the business entity page of the application complete fingerprint background checks.

Plan of Correction: The three applicants and three agents listed on the business entity page of the application will complete fingerprint background checks. They will have scheduled an appointment within five days.
