Comments:
An unannounced subsidy health and safety inspection was conducted on 08/07/2019 from 10:50am - 1:10pm. At the time of entrance there were 41 children in care with five teaching staff and the program director. The children ranged in age from two years - school age. A monitoring inspection was concurrently conducted as this facility is a licensed child day center. Children were observed during morning program time and lunch. Records reviewed for eight children and one new staff. The violation is listed on the violation notice and was discussed with the program director at the conclusion of the inspection.

Description: Based on record review and interview, the vendor failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional. Evidence: 1. Child 1 was injured on 07/26/2019 while on a facility sponsored field trip. The parent of child 1 took her child to an urgent care facility on 07/29/2019 according to written information provided by the center director. 2. This injury was reported to the inspector by the center director during the monitoring inspection conducted on 08/07/2019 which is outside of the two business day time frame for reporting injuries requiring treatment from a medical professional.

Plan of Correction: The on-line injury reporting form will be completed today. Going forward, all injuries requiring outside medical attention will be reported to the Department of Social Services within two business days.
