Technical Assistance:
The schedule and curriculum in the school age classroom needs to be followed to ensure children are not bored and that they are provided needed structure to lessen the likelihood of the schoolagers engaging in problematic behaviors. You may want to consider separating the older and younger children within the classroom into two separate groups which may assist in a more manageable execution of age appropriate activities.

Comments:
The center self-reported two situations involving care, protection and guidance and field trip permission authorization. Interviews, observation and a review of documentation, record keeping and files were conducted today and there were two violations resulting from this inspection. Please submit your plan of correction by July 23, 2019. Arrival time: 12 noon Departure time: 3:05 pm

Description: Based on observation, it was determined that medication was not kept locked. Evidence: The Licensing Inspector observed Albuterol medication for child B unlocked and sitting on the desk in the office.

Plan of Correction: Medication was moved immediately. Ensure medication is put away by the same person who brings it out.

Standard #: 22VAC40-185-580-L
Description: Based on interviews, it was determined that a child was transported on a field trip to a local pool despite not having parental permission for the field trip. Evidence: On 7/18/19 at approximately 8: 50 am the center transported a group of 14 schoolagers ( including child A )on a field trip to a local pool. Child A's parents had a conversation with the director and staff communicating that child A was not to go on the field trip. Consequently the parents did not sign parental permission for the trip to the local pool. The center realized child A was not to be on the trip and made plans to return the child to the center at approximately 9:50 am. At no time did child A get into the pool.

Plan of Correction: Retraining of entire staff on CSRs and supervision. A manager, Emily will sign off on every field trip CSR before leaving the building. Permission slips will be checked by Center Director and primary teacher.
