Comments:
An unannounced monitoring inspection was conducted on 08/07/2019. There were 68 children, ages 2 months-10 years, and 9 staff members present during the inspection. The inspector reviewed 6 children's records, 6 staff records and 3 medications during the inspection. The children were observed in the following activities: having breakfast, participating in free choice activities, participating in a learning project, and going outside to play. Proper hand washing and table sanitization were also observed. The inspector reviewed the outdoor playground, the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed the following with the staff: subsidy requirements, playground requirements, medication/over-the-counter skin products, children's allergies, shelter-in-place and children's behavior. Standard #: 22VAC40-185-280-G
Description: Based on observations and discussion with staff, the facility failed to ensure that when hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents. Evidence: Staff #5 was observed spraying the contents of an unlabeled plastic spray bottle onto the tables after breakfast. Staff #5 verified that the bottle contained a soap and water solution.

Plan of Correction: The bottle was labeled during the inspection. The bottles will be inspected every few days to ensure they are labeled.

Standard #: 22VAC40-185-290-3
Description: Based on observation, discussion with the staff and record review, the facility failed to ensure that electrical outlets shall have protective covers in areas used by children of preschool age or younger. Evidence: There was an uncovered electrical outlet in a classroom that was being utilized by preschool aged children. Staff #7 verified that Child #2 was the youngest child in the class. The record for Child #2 confirmed that the child was 3 years old.

Plan of Correction: The outlet will be covered today.

Standard #: 22VAC40-185-540-E
Description: Based on observations and discussion with the staff, the facility failed to ensure that all required nonmedical emergency supplies are maintained on-site. Evidence: There was not a working battery-operated radio on-site during the 08/07/2019.

Plan of Correction: A new radio will be purchased today.

Standard #: 22VAC40-185-560-G
Description: Based on observations and discussion with the staff, the facility failed to ensure that when food is brought from home, the food container shall be clearly dated and labeled in a way that identifies the owner. Evidence: There was an unlabeled, undated lunch box in the infant room that contained food for two of the children enrolled in the center.

Plan of Correction: The lunch box was labeled and dated during the inspection. The administrator will make sure that the staff will labeled and dated each day.
