Comments:
An unannounced monitoring inspection was conducted beginning at 10:20 a.m. and ending at 1:00 p.m. with the director and 11 staff supervising 62 children ages 5 years through 11 years old. Kindergarten and first graders were observed on the side playground, third through fifth grade was on the back playground and second graders were inside working on an arts and craft project. After outdoor play, children transitioned inside and had bathroom and handwashing and prepared for lunch. Lunch today was choice of cheese or pepperoni pizza, sunwise sandwich or yogurt and fruit bread, baby carrots with dip, spiced apples and choice of milk. Seven children's records and five staff records were reviewed. Seven medications and authorizations were reviewed. Emergency drills are posted and up to date. Violations:
Standard #: 22VAC40-185-340-A
Description: Based on observation, not all staff ensured the care, protection and guidance of children in care. Evidence: On the playground, 23 third, fourth and fifth grade children were observed having free play being supervised by 4 staff members. One staff member was positioned on top of the horizontal climber and at least five children were observed using the playground climbing equipment in a manner in which it was not intended including climbing and standing on top of the two horizontal climbers, doing flips over and off the horizontal climber, doing flips off the climbing ladder/stairs to the horizontal climber and laying on top of the horizontal climbers. Two different staff were seen talking with staff member on top of the horizontal climber. No staff redirected this behavior or activity.

Plan of Correction: The staff member was spoken to during the inspection by the director on duty. All staff will be reminded of appropriate use of the equipment and active supervision.
